Mailmerge Word 2007: with a personalised link that enters an unique value to a database

i using mail merge in word (combined excel) create personalized emailing. i to add a link that when clicked upon the unique id-number of person is automatically added to database.

i including link pre-populates form in google documents person's id-number (i have no programming experience , way can use google database).

problem link creates, google.com link. there (simple) way make link doesn't google.com instead mydomain.com?

unfortunately, there no option link google document, may use excel/access store data if helps.
